A Tucson resident may request an application by calling the TOP DOG office or click here to download information and an application. Once the application is returned, a series of interviews are arranged. If the applicant has a dog, TOP DOG will conduct two interviews--one in the applicant's home and another in public to determine the suitability of the applicant and his or her dog for the program.

Part of the application is a medical form to be filled out and signed by the applicant's physician. Also included is a veterinarian form to help TOP DOG assess the dog's physical well-being and ability to withstand the rigors of service dog work.
